MEET ME @7 - Diabetes Self-management Tool for Patients and Caregivers

This application is designed to assist Diabetes patients in learning and performing their daily Diabetes self-management tasks. This application assists both Type 1 and Type 2 Diabetes patients in managing their diabetes.

This mobile application is based on the Diabetes self-management tool: MEET ME @ 7© and is designed to complement the medical care a patient receives from their healthcare team. Use of the MEET ME @ 7© application can help the patient to organize their diabetes self-care tasks, as well as create patient specific goals to improve their diabetes management.
